Preparing for Your First Visit
If you have not yet been seen at Kitaj Headache Center, please complete the new-patient forms before your appointment and bring the completed forms with you.
Please print the forms single-sided. Please arrive 15 minutes early for your first appointment so you can be registered.
New Patient Forms
The new-patient packet includes informational forms as well as forms that must be completed by all new patients. Download the packet, print it, and fill it out before your visit.
Need the Forms Mailed?
If you do not have access to a computer and printer, please call the office and ask to have the forms mailed to you.
